Outside Referrals

Thank you for your interest in the Denver Health Bariatric Surgery Center. We are excited for you to make this commitment, and we are here to support you along your journey.

You must have a primary care provider to submit the following bariatric referrals:

1. Bariatric surgery
2. Bariatric nutrition

Please provide the following documentation:

1. Health history with co-morbidities (problem list)
2. Medications
3. Past surgeries
4. Relevant labs
5. Attempts of weight loss
6. Current height, weight and BMI
7. Medicaid patients – please provide documentation with a qualifying BMI for at least the previous two years.
8. Medicare patients – please provide documentation with a qualifying BMI for at least the previous five years.
