This online care navigator assists THRIVE partners in connecting their clients to care across our community.

Adult Crisis Services

Mobile Crisis Services – Denver Health

Mobile Crisis Services responds in the community to individuals experiencing a behavioral health crisis, including persons with suicidal ideation or in need of hospitalization. Responders include licensed clinicians and peers; mobile crisis can place MHHs if needed. Response time is 1-2 hours.

Instructions for clinicians: Contact mobile crisis at 303-602-7224 (professionals only). 
Instructions for patients: Patients may access mobile crisis services themselves through 988. 

STAR Van – Denver Health/WellPower

STAR provides person-centric mobile crisis response to community members who are experiencing problems related to mental health, depression, poverty, homelessness, and/or substance use issues. Responders are social workers and paramedics. Call for estimated response time, which may vary based on call volume.

Psychiatric Emergency Services – Denver Health

The PES provides 24/7 assessment and management of all behavioral health emergencies, including suicidal ideation, violent ideation, and substance-related emergencies. Patients see a psychiatrist and are able to access inpatient hospitalization, intensive outpatient, or other care as needed. Patients will be triaged and receive medical services as necessary.

Funding/payor eligibility: no restrictions 
Instructions for clinicians: Patients can self-present or be transferred via STAR, mobile crisis, police, or ambulance. Contact the PES at 303-602-7221 to provide sign out and collateral information. 
Instructions for patients: Patients may present via a crisis service or by self-presenting to the main Denver Health ED at 777 Bannock St, 80204. 

Denver CARES

Denver CARES provides 24/7 withdrawal management and long-term residential treatment programming for persons with substance use disorders. Patients may be intoxicated or require withdrawal management; patients who are referred and require additional care may be transferred to Psychiatric Emergency Services at Denver Health.

Client eligibility and referral requirements: Ages 18+ 
Funding/payor eligibility: no restrictions 
Instructions for clinicians: Patients can self-present or be transferred via STAR, mobile crisis, or police. Contact CARES at 303-436-3500. Patient transport for impaired individuals is available from the emergency service patrol van at 720-913-2200. 
Instructions for patients: Patients may present via a crisis service or by self-present for treatment at 1155 Cherokee St., 80204.

SOAR – Seeing Our Adolescents Rise

SOAR provides individual therapy, coaching/mentoring, group adventure therapy, case management, and educational support and advocacy for youth struggling with mental health difficulties and justice involvement. SOAR can provide treatment in Spanish and in the community for clients

Client eligibility and referral requirements: Ages 7-18; clients should be willing to participate in weekly groups or have a SOAR therapist; SOAR is not ideal for individuals with psychosis or intellectual disabilities 
Funding/payor eligibility: Medicaid or self-pay; contracts with other organizations like PSYP and Pre-Trial services 
Instructions for clinicians: Contact SOAR via email (referrals@soarcolorado.org) or phone (720-675-7761). We reach out as fast as we can depending on staff availability. We will contact clients via phone call and text message. Services start as soon as there is staff availability. Group availability is limited. 
Helpful tip: Let the family know that we are a community organization that relies heavily on communication the day prior to and day of services. If we do not get confirmation from clients the day of, we do not drive to them, as we serve across the Denver Metro area. 

Please provide us with as much context as possible surrounding family barriers to services and potential needs. 

Instructions for patients: Inform clients to expect a phone call and/or text message from a staff member from SOAR. Clients need to return the call or text to start services. 

Denver Health STEP Addiction Treatment Program for Youth (up to 21)

Denver Health STEP specializes in substance treatment for adolescents up to the age of 21. Individual, family, group, intensive outpatient and in-home therapy are available. Psychiatric services, contingency management, and medication assisted treatment are available. STEP can facilitate entry into withdrawal management and coordinate care with other programs.

Client eligibility and referral requirements: Ages 12-21, youth must have a substance use disorder 
Funding/payor eligibility: no restrictions for Denver County residents; residents out of county must have health insurance 
Instructions for clinicians: Call 303-602-4848, Monday-Friday, 8am-6pm and/or email OBHS_STEP@dhha.org 
Helpful tip: Clients will need to participate in individual therapy to receive medication management or IOP services. Please notify intake line if you are interested in In-home, MAT services, or IOP group. STEP hours are Monday-Friday, 8am-6pm. 
Instructions for patients: Call or email us. Please provide the name of the client and the best call back number. After the referral, the STEP team will make two attempts to contact youth within two business days. Clients who do not respond after two attempts may still call the above number for an intake.

Second Chance Center

Second Chance Center offers reentry services including peer mentoring, care management, employment services, housing opportunities, behavioral health services, and refuse. Second Chance Center focuses on formerly incarcerated adults, persons with serious and persistent mental illness, individuals experiencing homelessness or substance use disorders.

Tribe Recovery

Tribe Recovery offers 30-day residential treatment for me; intensive outpatient for men and women; outpatient services; DUI; peer support; and additional programs for persons who have a history of incarceration or homelessness or are pending incarceration.

Client eligibility and referral requirements: Ages 18+ and have a substance use issue. Cannot serve those with a history of sexual or arson crimes or severe mental health disorders (such as primary psychosis) 
Funding/payor eligibility: Must have Medicaid 
Instructions for clinicians: Providers should communicate that Tribe is a program of recovery where participants actively participate in clinical services. Services begin upon intake into our homes. Clients begin the process of intake with a prescreen over the phone with our intake team.

Denver Health Outpatient Behavioral Health and Addiction Treatment for Adults

Denver Health offers comprehensive substance treatment programming, including intensive outpatient services and rapid referrals to residential and withdrawal management programs. Clients can receive evidence-based treatment including medication management for relapse prevention, contingency management for methamphetamine addiction treatment, and access to medical services. Peer support is integrated into services.

Client eligibility and referral requirements: Ages 18+ (for youth up to age 21, see separate entry for youth addiction treatment at Denver Health’s STEP program)

Instructions for patients: Individual walk-ins are available at the Second Chance Center from 8 am - 4 pm Monday through Thursday and 8:30 am - 1:20 pm on Saturday for those wishing to engage in re-entry services.
