Important Information About Accessing Your Child's Medical Records

There has been a recent policy change to MyChart at Denver Health. The minimum patient age for an active account is now 12. Parents or guardians who wish to still view their minor’s record through proxy access (the adult does not have to be a patient at Denver Health) should fill out the required form titled “MyChart Child (under 12 years of age) Proxy Access Request Form”. Once complete, please submit all required documentation to Denver Health HIM department via fax, e-mail, or traditional mail found on the bottom of the form. Questions? Call (303) 602-8000.
