Who can request medical records?
The Health Insurance Portability and Accountability Act, also known as HIPAA, requires hospitals to obtain an original, complete and properly executed Authorization for Disclosure of Health Information form in order to provide a copy of a patient's medical record to a patient or anyone he or she designates.
Only the patient, parent/legal guardian or the patient's legal Health Care Representative can sign the form to release medical records.
The authorization form must be legible and complete. Authorizations that are illegible or incomplete will be returned. Requests for information regarding any of the following items must also be specifically initialed on the form:
- Diagnosis and/or treatment relating to mental health conditions;
- Diagnosis and/or treatment relating to HIV testing, infection or diagnosis and/or treatment for AIDS
Fees
The cost is $14.00 for the first 10 pages, 50 cents per page for pages 11-40, and 33 cents for each page over 41. Copies of records are sent to medical facilities or other physicians at no charge.
Request Records
Please fill out the release form and mail, fax or deliver it in person to request copies of your medical records.
Records are usually available within 5 to 10 business days from the time the request is received.
If you are interested in picking up your medical records in person, when they become available, please note this at the top of the request form. If you plan to come to the Medical Records department to pick up a request that you have submitted, you must have a photo ID with you.
