Improving Asthma Care in an Integrated Safety Net through a Community Available Electronic Medical Record

The primary purpose of this project is to improve the quality and patient-centeredness of ambulatory asthma care for children and adults by establishing a link between existing health information technology applications. 

One application is publicly available, online interactive asthma action plan (IAAP). The other is a commercially available electronic medical record system that integrates a health system's functionalities. Currently, the two applications do not communicate with one another, so a provider who wishes to take advantage of the decision support and patient-centered output available through the IAAP must leave his/her electronic medical record session in order to do so.

This project will establish a link between the two extant applications, improving information flow and efficacy in the process, adding value to both systems.
