As one of the largest community health center networks in the nation, our providers see a diverse patient population, and treat many of the health conditions prevalent in our community. In fact, 25 percent of adults in Denver are seen in our community health centers, and 35 percent of children in Denver call our family health centers their medical home. Our providers are experienced, caring, and have all of the resources in place to provide you with the best medical care, so you stay healthy and well.
With eight community health centers and 16 school-based health centers located throughout Denver, we are sure to have a convenient option for you and your family. Additionally, with each of our community health centers offering pharmacy services on-site, getting your prescriptions is also convenient and easy.
The community health services network serves as a national model of service delivery and its high-quality care has garnered national awards, including The Shingo Prize Bronze Medallion for Operational Excellence, an award that recognizes our use of process improvement in providing care and excellent customer service to our patients.
Denver Health’s Community Health Services has a rich history of serving Denver. Beginning in 1966, with the opening of the Eastside Family Health Center, the nation’s first family health center west of the Mississippi River, we have had a strong commitment to the communities we serve and the patients that have walked through our doors over the years. We have several providers who have cared for generations of families in Denver. What began as a single clinic, housed in a former bakery, has transformed into a network of community health centers, hospital-based urgent care centers for adults and children and school-based health centers.
