You can obtain birth and death certificates in a variety of ways:
You will be asked to complete a form and to present your current driver's license, state identification, passport, or other form of identification that is on the list of accepted identification.
It will take 10 to 40 minutes for us to process your request.
You must arrive before 4:00 PM, to be processed by closing.
To request a certificate by mail, please fill out the Birth Certificate request form or Death Certificate request form and return the signed application, fee and photocopies of the appropriate proof of identity or relationship to our office.
It is best to use the request form, but if you are unable to print the application, please send a written request including the following:
- Your full name
- Date and place of birth
- Parents' names (including mother's maiden name)
- Your daytime phone number
- Your return address
- A photocopy of your current, valid state-issued driver's license, state identification card, passport, or other acceptable identity document(s)
Mail requests will require four weeks processing time after we receive it.

Your order will be processed through and independent company that we have partnered with to provide you this service; VitalChek Network, Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express, Discover, MasterCard or Visa.
Phone requests are processed in three to five business days and have an additional $9 processing fee.
Next day service is available for an additional $19 processing fee.
