Travel Consultation Appointments

Preparing for Travel Consultation Appointments

  • Travel consultation appointments should be scheduled six to seven weeks prior to your departure date. However, every attempt will be made to accommodate short-notice requests.

  • Please arrive 15-20 minutes before the scheduled travel consultation.

  • Patients between the ages of 6 months and 9 months of age or over 60 years of age will be REQUIRED to make a travel consultation to receive the Yellow Fever Vaccine due to a possibility of side-effects. All patients receiving Yellow Fever vaccine will complete a health questionnaire.  If there are any concerns raised from the questionnaire responses, the patient, at any age, may be required to schedule a travel consultation prior to receiving a Yellow Fever vaccine.

  • Bring your trip-specific itinerary, completed Traveler Registration & Medical Assessment Form, a copy of your previous immunizations with dates, and your International Certificate of Vaccination (yellow card), if you have one.

  • To facilitate your admission process, please print out a copy of the Comprehensive Medical Evaluation Form. Fill out the form to the best of your ability and bring it with you to your travel consultation. If this is not possible, bring your medical information which includes: medication allergies, current prescription medication, and medical diagnoses.

  • Individual travel consults are approximately 30-45 minutes in length. Allow for 60-90 minutes to complete the entire process, which includes administrative paperwork and vaccinations.

  • There are no restrictions on food, beverages or activities prior to or after your appointment. We do encourage you to drink and eat before your appointment.

  • We do not accept health care insurance plans. Payment is due upon services rendered, and we accept cash, check, Discover, MasterCard or VISA. A copy of the charges and a billing form will be provided if you wish to pursue reimbursement.

  • A Travel Consultation by appointment, which includes Malaria prescriptions, is $75 for the first person in the family/group and $50/each additional adult/child plus the cost of immunizations. For families with more than two children - $25 each additional child plus the cost of immunizations

  • We ask you to come early for your appointment and come prepared. We ask that only travelers (children & adults) attend the travel consultation and that you leave other small children at home.
